More than 200 primary care practices are located throughout Vermont, including practices owned by individuals, groups and hospitals. There are also more than 350 individual and group dental practices. To become a patient or an employee of any of these practices, follow one of the links below or do a simple online search to find contact information for the practices of interest.
Many of these practices are Federally Qualified Health Center (FQHC) sites or Rural Health Clinics (RHCs), and are considered part of the health care safety net. Under Medicare and Medicaid rules, safety net providers are required to provide health care services to all Vermonters, regardless of insurance status and mostly with a sliding fee scale.
Most practices in Vermont include a mix of physicians, physician assistants and nurse practitioners, and are certified patient-centered medical homes.
