The Vermont Department of Health Diabetes Program is funded through a Centers for Disease Control and Prevention (CDC) grant. 

Program Goals

The goal is to improve the health of Vermonters by preventing and managing type 2 diabetes and heart disease. A focus of our work is using proven methods to support Vermonters who are at the greatest risk of these conditions.

  • Increase the number of Vermonters who are screened for prediabetes or diabetes and referred to resources.
  • Promote and connect Vermonters to free, statewide My Healthy VT Diabetes Workshops for diabetes. 
  • Address the conditions where people live, learn, work, and play to lower the risk of type 2 diabetes. 
  • Support and empower community health workers.

Current Projects

Quality improvement 
We work with several federally-qualified health centers, hospital-owned practices, and Support and Services at Home (SASH) sites across the state to identify and treat people with diabetes more effectively. Learn More

Statewide Community Health Worker infrastructure
There is strong evidence that community health workers can improve health outcomes for people with prediabetes and diabetes. Our program provides the backbone support for a group of organizations working towards bringing the community health worker workforce together.  Learn More

Pharmacist collaborations 
We are building relationships with pharmacists to support the use of Medication Therapy Management (MTM) and other services for diabetes and heart disease. Medication Therapy Management is a service that helps people to better understand their health conditions and the medicines used to manage them.

Self-management workshops
We coordinate with state level health leadership, including the Vermont Blueprint for Health and the University of Vermont Medical Center's Community Health Improvement, to promote and recruit participants for the My Healthy VT Diabetes Prevention and Diabetes Management Workshops. We aim to make it easier for people of all abilities to participate in diabetes workshops. Learn more about My Healthy VT Workshops
